Before going Scuba Diving, it is necessary to complete a Medical Statement about your own health. This includes a questionnaire which you must complete giving full written YES or NO answers.
Click on the appropriate form below to download. You must print it out, complete it and bring it with you, or you will be asked to complete it here before you dive.
If you answer any question with “YES”, then we require your doctor to verify your fitness for diving – please consult your GP.
Should you not have enough time to arrange a consultation with your doctor, then we can arrange this for you here.
