Before going Scuba Diving it is necessary to make a Medical Statement about your own health. This includes a questionnaire which you must complete giving full written YES or NO answers.

If you answer YES to any of the questions we may need to make further checks before clearing you to dive.

Bubblemaker Statement
Bubblemaker Courses have their own specific Statement including health questions which must be completed before taking part in the course. It must be signed by a Parent or Guardian.

Scuba diving has inherent risks and to ensure you are aware of these before we take you diving you will be asked to complete this form.

PADI IE Schedule
The PADI Instructor Exam which is taken upon completion of the Instructor Development Course is run by a PADI Examiner on this schedule. This schedule is subject to change and we accept no liability in that eventuality.
